Cellulite refers to fat deposits under the surface of the skin giving it a dimpled, irregular appearance. The vast majority of women have cellulite, some estimates are as high as 90 percent. Cellulite is routinely found in the thighs, hips, buttocks, and stomach. Unfortunately, maintaining an optimal body weight, eating healthy, staying active, and even regular exercise may not be enough to combat cellulite.

Research on the mechanobiology (the science that integrates mechanics, molecular biology and genetics) of the skin shows that deep mechanical massage will induce changes in the dermis connective tissue (Silver, Siperko, and Seehra, 2003). However, in review of the published research on endermologie, which is minimal, this device does not show meaningful efficacy in the treatment of cellulite (Avram, 2004; Van Vliet, 2005).

Serums and topical creams can help, but they aren’t going to perform any miracles. At best, they can help strengthen the skin, making your cellulite less visible and even temporarily tighten it with ingredients like caffeine. Two Engelman recommends: Nerium Firming Body Contour Cream, $120, which has caffeine, forskohlii root extract, and antioxidants — all which help tighten and firm skin. And Biotherma Celluli Eraser, $45, which has caffeine and coralline extract, a marine algae extract that helps minimize fat cells.

Topical treatments may help to reduce the visibility of cellulite, but the effects may not be long-lasting. You’ll need to apply the product daily to maintain results. Creams are said to tighten the skin, which makes the skin appear smoother and firmer. They also add moisture, which can reduce the appearance of cellulite.

One of the most recognized benefits of grapefruit essential oil and other citrus oils is their positive effect on the lymphatic system. Using grapefruit oil may help boost the activity of lymph glands, thereby preventing problems like poor circulation and cellulite.22

Erythema nodosum is a skin inflammation that results in reddish, painful, tender lumps most commonly located in the front of the legs below the knees. Erythema nodosum can resolve on its own in three to six weeks, leaving a bruised area. Treatments include anti-inflammatory medications and cortisone by mouth or injection.

Some non-scientific sources have suggested that cellulite is a lymphatic disease, yet there is no scientific support for this contention. However, limitations to fluid movement and drainage (as seen with inflammatory skin conditions) may contribute to the appearance of cellulite.

I am talking about cellulite, which some 80 to 90 percent of women — even Olympic beach volleyball players and supermodel yogis — have. While those oh-so-attractive lines and divots in your fatty bits are in no way harmful to your health, the dermatologists and plastic surgeons who study the condition have nonetheless staged its severity just as they have cancer. From what I can tell, my butt is between stages I and II: a bit of visible orange-peel texture when I am standing, which is more obvious when I clench or grab a chunk (sorry — TMI!). Cellfina, another FDA-approved treatment, promises to reduce cellulite in a single session. Your doctor will numb the area then insert a needle-sized device in your skin. He’ll slice through the cellulite-causing bands under your skin that cause the lumps and bumps.  The changes should last about 2 years. Cellfina doesn’t work if you have loose skin or so-called “orange peel” cellulite.

Twenty adult women (ages 25-57 years) of various skin phototypes (I-V), and with moderate bilateral thigh and buttock cellulite, received eight biweekly treatments to a randomly selected side (the contralateral side serving as a non-treated control). A combined bipolar radiofrequency, infrared light, and mechanical suction-based massage device was applied at 20 watts RF, 20 watts IR (700-1500 nm) light, and 200 millibar vacuum (750 mmHg negative pressure). Patients were evaluated using standardized digital photography and circumferential leg measurements at baseline, prior to each treatment session, and at one, three, and six months after the final treatment. Clinical improvement scores of comparable photographs using a quartile grading scale (0 = or < 25%, 1 = 25%-50%, 2 = 51%-75%, 3 = or >75% improvement) were made independently by two masked medical assessors after the series of treatments. Final post-treatment body weight and patients’ subjective evaluations were recorded.

Alternative or supplemental therapies include caffeine, grape seed extract, or gingko biloba. These agents have been applied topically, orally, and by injection, but none of them have proven effective.
